Patient Referral Process
- Reach out.
- We'll help your patient find their place.
- Make a plan.
- And take their next step.
At Veritas Collaborative, we envision a world in which all persons with eating disorders, their families, and their communities have access to best-practice care and hold hope for a cure. It is something we spend a lot of time thinking about, but we recognize it may not be something that you or your patient has a lot of experience with. We are here to partner with you and be your patient's guide on the journey to recovery. We promise we know the way.
We want to hear from you! Whether your patient is ready to begin treatment as soon as possible or you want to learn more about our programs or eating disorders, we're happy to talk through the details. Give us a call or contact us online.
Help your Patient Find their Place
In order to assess if Veritas Collaborative is an appropriate fit for your patient, we need to collect some clinical and medical information. The more details we have, the better we can guide your patient along their recovery journey. Please take a moment to help your patient complete the forms to the right.
Insurance & Financials
Next, we will look into the details of your patient's insurance plan on their behalf and prepare an estimated financial projection for their review prior to admission. We believe in treating to outcome, irrespective of resource, and we will work diligently with your patient's insurance provider to help them understand the care your patient will need along the way.
We continue to accept the following insurance plans: Aetna, America's Choice Provider Network, Blue Cross Blue Shield, Cigna, ComPsych, Magellan, Multiplan, and Tricare. If you don't see your patient's carrier or have questions about insurance or coverage, we're here to help.
On Admissions Day & Beyond
We understand that planning for an admission is challenging and the first day of treatment may seem scary to your patient, their family, and their community.
We're here to help ease that stress and make the admission day flow as smoothly as possible. Prior to admission, your patient's Clinical Intake Coordinator will provide them with a packing guide and an admission day agenda. They will also answer any questions they have about the admission process and our treatment programs. If they're not from the area, our team can also help navigate local resources and assist their family or community members with procuring housing and other services.
When they arrive on admission day, they will be greeted by the Clinical Intake Coordinator with whom you all have been working — or another dedicated member of our team — who will assist them each step of the way.
During their admission, they will complete the necessary paperwork, have the opportunity to tour the facility if medically appropriate, and briefly meet with each member of their core treatment team.
Their multidisciplinary treatment team will remain with them throughout their journey at one of our healthcare system locations and will soon grow to include their family and/or community. As they step down to lower levels of care along their journey to recovery, we recognize that it may be better for them and for those important to them if they were closer to home. For this reason, we offer a full spectrum of levels of care in our different locations in North Carolina, Virginia, and Georgia. By providing a complete continuum of care, we are able to position them for long-term success in recovery.
Additionally, and with their permission, we will keep you and their other outpatient treatment providers updated on their progress and, when the time comes, work with you to ensure that you have the resources you need to help your patient maintain recovery in their home environment. Our partnership with you begins when you reach out to a member of our team, but it does not end when your patient walks out our doors.
